Applying

Follow the following steps in applying for CSAO Reassessment:

  • Complete the CSAO Reassessmet Form (included with your CSAO Performance Results Report)
  • Read the CSAO section of this website in its entirety to understand the CSAO and CEO’s policies and procedures, and to ensure that you are eligible to take the CSAO. 
  • For a current list of designated individuals to authorize candidates’ identity and eligibility for ACOE accredited schools and UWIOBP, please click here.
  • Attach a verified passport-style photograph for identification.
  • Enclose your cheque for the appropriate amount (Canadian funds only) made payable to Canadian Examiners in Optometry.
  • Sign the form indicating that you agree to the terms and conditions of the Assessment.

Please Note: If you are sending CSAO registration documents via courier from outside Canada, you are advised NOT to assign a declared value. Declared values are subject to duties and taxes for which CEO will not be responsible. As a result your delivery may be declined at point of clearing.

Confirmation of your candidacy will be sent to you upon payment of your application fee.

The address given on your application form will be the address to which CEO directs its communication with you. CEO is not responsible for any miscommunication as a result of you not providing a change of address notice, in writing, with complete details of all changes.

The application deadline date is never extended. Ensure that CEO receives your completed application form on or before the deadline date. A valid cheque or money order must accompany your application form for it to be processed. Incomplete application forms or applications without appropriate payment will not be processed.

Please note that jurisprudence examination is specific to the individual provincial jurisdiction to which you are applying. Although CEO may administer jurisprudence examinations on behalf of those provincial regulators requesting this service, the responsibility for the exam remains with each respective regulator and you must contact the regulator to register and/or to obtain further information regarding the jurisprudence process.
