CSAO > Applying > Eligibility
To be eligible to take the CSAO, you must provide
valid documentation that you have
graduated with a Doctor of Optometry degree from a program accredited by the Accreditation Council on Optometric Education (ACOE)
or
will graduate with a Doctor of Optometry degree from a program accredited by the ACOE within sixty days of the date of the CSAO administration for which you are applying
or
successfully completed the International Optometric Bridging Program of the University of Waterloo
In order to confirm CSAO eligibility, the following information must be completed on
the CSAO Application Form with applicable documentation and verified photo identification:
I am the Director/Dean, Undergraduate Officer, or authorized representative of the academic institution indicated below (on registration form) and this individual is currently enrolled in the final year of the optometry program at same and is anticipated to graduate with a Doctor of Optometry degree: within sixty days of the date of the CSAO administration for which you are applying.
or
I am the Director, or authorized representative, of the UWIOBP and this individual is currently enrolled in, and will have completed, the UWIOBP program prior to: the CSAO administration for which you are applying.
or
I am the State Registrar, or equivalent presiding officer, of the regulatory board as stated below (on registration form) and this individual is licensed, or registered, and practicing in good standing within the jurisdiction of said regulatory board.
or
I am an authorized notary public and this individual is a non-licensed optometrist with a Doctor of Optometry degree from an ACOE accredited optometric program. As further evidence, I have notarized copies of this individual’s optometric degree certificate and final year transcripts.
